  • Troponines T hs et BNP → facteurs pronostics = répercussion hémodynamique sur le coeur droit
  • Angio-CT thoracique en protocole embolie pulmonaire

Traitement

Anticoagulation thérapeutique

  • Anticoagulants parentéraux : HBPM, fondaparinux > HNF
  • Anticoagulants oraux directs
  • Anti-vitamine K : When VKAs are used, anticoagulation with UFH, LMWH, or fondaparinux should be continued in parallel with the oral anticoagulant for >= 5 days and until the international normalized ratio (INR) value has been 2.0-3.0 for 2 consecutive days[2].
  • Embolie pulmonaire provoquée: anticoagulation durant 3 mois
  • Embolie pulmonaire non provoquée chez patient à bas risque de saignement, facteurs de risque irréversibles: discuter anticoagulation au long cours

Thrombolyse

  • Embolie pulmonaire massive avec choc
  • Embolie pulmonaire submassive avec facteurs de mauvais pronostic (troponine T hs et BNP élevés, échocardiographie évocatrice)

Filtre cave

  • Pour les patients chez qui une anticoagulation est impossible
