Anévrisme de l'aorte thoracique
- Définition: augmentation du diamètre de l'AT > 50% de la norme ou de la valeur prédite
- Facteurs de risque: bicuspidie aortique (environ 50% développent un AAT)
- Dépistage: Screening for abnormalities of the thoracic aorta with aortic imaging is indicated in asymptomatic patients with genetic conditions (such as Marfan or Ehlers-Danlos syndrome), a bicuspid aortic valve, or a family history of TAA or aortic dissection. Screening is not recommended in other asymptomatic persons.
- Suivi:
- AAT et bicuspidie aortique: Patients with a bicuspid aortic valve and a thoracic aortic aneurysm should undergo echocardiography every 6 months:
- if the aortic diameter is larger than 4.5 cm
- or the rate of enlargement exceeds 0.5 cm/year.
- AAT et bicuspidie aortique: Patients with a bicuspid aortic valve and a thoracic aortic aneurysm should undergo echocardiography every 6 months:
- Traitement:
- Chirurgie prophylactique recommandée:
- si diamètre aortique > 50 mm (> 45-50 mm si syndrome de Marfan)
- si diamètre aortique > 45 mm et chirurgie cardiaque concomitante
- croissance rapide > 5 mm par année
- AAT et maladie coronarienne/valvulopathie: In patients with a thoracic aortic aneurysm greater than 4.5 cm in diameter who require coronary artery bypass graft surgery or surgery to repair valve pathology, aortic repair should be performed at the time of cardiac surgery.
Anévrisme de l'aorte abdominale
- Définition: augmentation du diamètre de l'AA > 3 cm
- Dépistage:
- Suivi: Patients with an abdominal aortic aneurysm smaller than 5.5 cm should undergo surveillance ultrasonography, with frequency determined by aneurysm size.
