Hypokinésie

Maladie de Parkinson

  • Diagnostic:
    • Signes cardinaux → the diagnosis of Parkinson disease requires the presence of bradykinesia and at least one other cardinal feature and the absence of red flags for atypical forms of parkinsonism
      1. tremor de repos
      2. bradykinésie
      3. rigidité
      4. trouble de la démarche et de l'équilibre
    • Signes non évocateurs d'une maladie de Parkinson → red flags[1]
      • Chutes précoces (>1/an pendant 3 premières années)
      • Dysautonomie précoce, syndrome cérebelleux, syndrome pyramidal, troubles oculo-moteurs
      • Troubles cognitifs précoces (avant signes moteurs ou dans la première année suivant le diagnostic)
      • Syndrome parkinsonien symétrique prédominant sur les membres inférieurs
  • Traitement:
    • Dopamine agonists are the preferred initial first-line medication in patients younger than 65 years and levodopa is the preferred initial therapy in older patients.
    • Deep brain stimulation is indicated for patients who continue to benefit from levodopa but experience severe motor fluctuations or have a refractory tremor.
  • Complications médicamenteuses:[2]
    • Dyskinésie de pic de dose → choréique, moitié supérieure du corps, au pic pharmacocinétique
    • Dystonie de fin de dose → dystonie tonique du membre inférieur, hallux striatal (pseudo-Babinski)

Syndromes parkinsoniens atypiques

Paralysie supranucléaire

  • Caractéristiques: Patients with progressive supranuclear palsy have parkinsonism, impairment in both ocular range of movement and saccades (very fast jumps from one eye position to another, particularly in the vertical direction), facial dystonia, axial rigidity, and prominent postural instability with early falls.

Atrophie multisystémique

  • Caractéristiques: Multiple system atrophy presents with a combination of parkinsonism, ataxia, and severe dysautonomia and also is associated with early falls.

Dégénérescence corticobasale

  • Caractéristiques: Corticobasal degeneration often presents with markedly asymmetric parkinsonism and frequently is associated with dystonia, myoclonus, cortical sensory deficits, cognitive deficits, and apraxia (impaired motor planning).

Tremor physiologique

Tremor essentiel

  • Caractéristique: tremblement des 2 membres supérieurs
  • Traitement: The treatment of essential tremor is symptomatic; the most effective pharmacologic agents are propranolol, primidone, and topiramate.

Syndrome des jambes sans repos

  • Bilan: dosage de la ferritine → hypoferritinémie (15-75 μg/l) peut provoquer RLS
  • Traitement: agoniste dopaminergique (pramipexol)
