Hyperthyroïdie

- Bilan: anti-TRAc
Hyperthyroïdie subclinique
- Traitement:
- si TSH < 0,1 μU/ml → méthimazole
- si TSH > 0,1 μU/ml → répéter les tests thyroïdiens dans 4−6 mois
Hypothyroïdie
- Plus fréquente: thyroïdite de Hashimoto
- Bilan: anti-TPO
Hypothyroïdie subclinique
- TSH au-dessus de la limite supérieure de la norme
- T4 normale
- The normal range for TSH increases with age, and a TSH level of up to 10 µU/mL (10 mU/L) is within the normal range for persons 80 years of age and older.
- Si TSH > 10 mUI/l et élévation du LDL-cholestérol:
- Prévention maladie cardiovasculaire et insuffisance cardiaque

Treatment for subclinical hypothyroidism with TSH less than 20 µU/mL (20 mU/L) should be considered in younger patients, those attempting to become pregnant, or if severe symptoms are present.
Substitution
- Si absence de contre-indications:
- Dose 1.6 mcg/kg
- Patients âgés, maladie cardiovasculaire:
- Dose max 25-50 mcg/j
- For women with hypothyroidism adequately treated with levothyroxine before pregnancy, dosing can be empirically increased by 30% when pregnancy is confirmed.
