Hyperthyroïdie

• Maladie de Graves-Basedow
• Bilan: anti-TRAc

Hyperthyroïdie subclinique

• Traitement:
  • si TSH < 0,1 μU/ml → méthimazole
  • si TSH > 0,1 μU/ml → répéter les tests thyroïdiens dans 4−6 mois

Hypothyroïdie

• Plus fréquente: thyroïdite de Hashimoto
• Bilan: anti-TPO

Hypothyroïdie subclinique

• TSH au-dessus de la limite supérieure de la norme avec T₄ normale
  • Répéter le dosage après 2-3 mois pour confirmer le diagnostic
• The normal range for TSH increases with age, and a TSH level of up to 10 µU/mL (10 mU/L) is within the normal range for persons 80 years of age and older.
• Si TSH > 10 mUI/l et élévation du LDL-cholestérol:
  • Prévention maladie cardiovasculaire et insuffisance cardiaque
• Si TSH > 20 mUI/l
  • Traitement d'office

Treatment for subclinical hypothyroidism with TSH less than 20 µU/mL (20 mU/L) should be considered in younger patients, those attempting to become pregnant, or if severe symptoms are present.

Substitution

• Si absence de contre-indications:
  • Dose 1,6 μg/kg (≈ 100 μg/jour)
• Patients âgés
  • Dose de départ 25−50 μg/j, ↑ 25 μg/j aux 6 semaines pour TSH cible 1−2,5 mUI/l
• Maladie cardiovasculaire
  • Dose de départ 12,5−25 μg/j, ↑ 12,5−25 μg/j aux 6 semaines pour TSH cible 1−2,5 mUI/l
• For women with hypothyroidism adequately treated with levothyroxine before pregnancy, dosing can be empirically increased by 30% when pregnancy is confirmed.

Coma myxoedémateux

• Traitement: In patients with myxedema coma, intravenous hydrocortisone should be administered before thyroid hormones to treat possible adrenal insufficiency.

Euthyroid sick syndrome

Synonyme: nonthyroidal illness syndrome

• Caractéristiques: ↓/= T₄ avec TSH supprimée initialement, puis ↑ TSH. Normalisation des tests thyroïdiens après 4-8 semaines.
• Traitement: pas de traitement !

Suivi

• Contrôle de la fonction thyroïdienne TSH à 6-8 semaines
