Maladie de Crohn
- Traitement:
- Induction de rémission: glucocorticoïdes → budésonide (mais pas pour maintenance)
- Maintien de rémission chez patients dépendants des glucocorticoïdes: méthotrexate
- Traitement de maintenance:
- Maladie de Crohn sévère: agents anti-adhésion → natalizumab (CAVE si JC-virus négatif) et vedolizumab
- Induction de rémission: glucocorticoïdes → budésonide (mais pas pour maintenance)
Recto-colite ulcéro-hémorragique (RCUH)
- Traitement:
- RCUH légère à modérée: 5-ASA (sulfasalazine, olsalazine, balsalazide, mésalamine CR)
- Si proctite ou atteinte du côlon descendant/sigmoïde/rectum: lavements ou suppositoires de 5-ASA
- Induction de rémission: glucocorticoïdes → budésonide (mais pas pour maintenance)
- Maintien de rémission chez patients dépendants des glucocorticoïdes: azathioprine, 6-mercaptopurine → épargne des glucocorticoïdes
- Traitement de maintenance: 5-ASA (sulfasalazine → nombreux effets indésirables, mais traitement de choix si présence de symptômes arthritiques)
